Scope
This notice applies to information submitted through the Praxium Health website, primarily through the exploratory-meeting form for physicians and residents. Do not submit medical information through this form.
Information collected
We may collect your name, email address, phone number, professional profile, specialty or program, expected practice start period, preferred clinic and comments.
How information is used and disclosed
This information is used only to process your request, send a confirmation, arrange the exploratory meeting and complete related physician-recruitment follow-up. All requests are sent to the person authorized by Praxium Health, regardless of the selected clinic. Local clinic representatives do not receive form notifications at this time.
Information is not sold or used for behavioural advertising.
Consent
The form is submitted only after you consent to being contacted about the meeting. You may withdraw your consent by writing to the address below, subject to processing already required for an earlier request.
Optional consent to receive SMS schedule notifications is separate from general contact consent. Its status (yes or no), the submission date and the page language are stored with the request. No SMS is sent unless this consent is granted. If you opt in, message frequency varies, message and data rates may apply, and you may reply STOP to opt out or HELP for assistance.
Retention
Requests are retained for no more than six months and are then deleted, unless a legal obligation requires a different retention period.
Security and limited access
Information is accessible only to authorized people involved in arranging or following up on the meeting. Praxium Health uses encrypted connections, server-side validation, restricted administration access and controls against automated submissions. No system can provide absolute security.
